During onsite inspection, Licensor observed an electric extension cord in the outdoor area capable of forming a loop unattended and accessible to children. (2)(c) During onsite inspection, Licensor observed a playpen in the licensed area placed directly under a window. (2)(g) During onsite inspection, Licensor observed several stumps and an unraveled garden hose in the outdoor play area posing a tripping risk. (3)(e) During onsite inspection, Licensor observed stairs leading to the second floor of the family home accessible and not equipped with a pressure gate, and a broken pressure gate at the bottom of the outdoor deck stairs.
